The Medical Society of the County of Erie is introducing three prestigious awards that aim to recognize outstanding contributions and dedication within the medical community. The awards will be presented at the MSCE Save a Life! Gala on October 10, 2024:

NOMINATION FORM

1. Lifetime Achievement Award

Description: This award honors a medical professional who has made lasting and significant contributions to the field of medicine throughout their career. It recognizes individuals who have demonstrated excellence in patient care, research, and medical education, significantly impacting the advancement of healthcare and medical science.

Eligibility Criteria:

1. Significant scientific contributions to medical knowledge and practice

2. At least 20 years of distinguished service as a medical professional

3. Demonstrated impact on patient care, medical education, or healthcare policy

4. MSCE membership is preferred but not required

2. Outstanding Service Award

Description: This award recognizes exceptional dedication and service within the medical field. It honors individuals who have gone above and beyond in their commitment to improving healthcare, advancing medical knowledge, and supporting their patients and colleagues.

Eligibility Criteria:

1. Outstanding contributions to patient care, medical research, or healthcare innovation

2. Significant impact on improving healthcare outcomes or medical practices

3. Notable leadership in addressing healthcare challenges or advancing medical ethics

4. MSCE membership is preferred but not required

3. Distinguished Mentor Award

Description: This award acknowledges a medical professional who has shown exceptional commitment to mentoring and guiding the next generation of healthcare providers. It recognizes the importance of nurturing talent and fostering professional growth within the medical field.

Eligibility Criteria:

1. Proven track record of mentoring medical professionals

2. Positive impact on mentees' professional and personal development

3. Commitment to fostering leadership and excellence in medicine

4. MSCE membership is preferred but not required
